Cystics Lesion of the Pancreas: EUS or ERCP?

Pancreatic cystic lesions have been increasingly recognized due to the wide spread us of cross-section imaging studies with various indications. Common pancreatic cystic lesions comprise pancreatic pseudocysts, and pancreatic cystic neoplasms including mucinous cystic neoplasms, and serous cystadenomas. ERCP have been used as the conventional endoscopic drainage tool in patients with pancreatic pseudocysts. Collaborating clinical contex and imaging studies is crucial for selection a good candidate for ERCP drainage. EUS could be used as a single-stop modality for drainage of pancreatic pseudocysts, particularly in those with non-bulging lesions and/or with portal hypertension. For pancreatic cystic neoplasms, EUS is currently considered as a standard approach mean given its ability for providing the better imaging resolution, and aspiration of cyst fluid analysis for differentiating between bening and pre-malignant/malignant lesions. ERCP may be used in a certain circumstances when the presence of pancreatic cyst-duct communication exists.
